This form is intended for faculty who wish to teach online/hybrid/web-enhanced courses using the WebCT Vista course management system. Please provide us with as much information as possible.
Requests for full online courses must be made no later than:
October 15 for courses taught in the Spring semester
February 15 for courses taught in the Summer semesters
April 15 for courses taught in the Fall semester
