Application and Reference Forms

Check back here in early in January 2010 for the application and reference forms.  You can also follow Kent State Accounting Scholarships on Twitter for notification about when the scholarship forms are available and for other announcements.

Application Information

1. This site contains information about how to apply for accounting major scholarships for the academic year of 2010—2011.  Complete and turn in all three pages of the application form (the link is listed above).

2. The application form MUST be typed.  It is in Adobe Acrobat (pdf) format and can be completed on your computer.  Once you are finished filling in the fields, print out the application.  Be sure to sign it on the bottom of the last page. Handwritten applications cannot be accepted.

3. All application materials are due to be turned in to Dr. Tietz by mid-February 2010 (check back here in early January for specific due dates.) Turn all materials in to:

4. Please include a transcript for all coursework completed (at KSU and at other schools attended) with your application materials.  The transcript does not have to be an official transcript; the transcript which can be printed using Flashline is sufficient.

5. Please include two references with your application (your references can send their reference forms to me separately also).  At least one reference must come from a KSU accounting professor whom you have had for class. The other ref­erence should be another professor (any field) or from a work supervisor.  The reference form link can be found near the top of this page.  You can also send the URL for this reference form to your reference persons; they can fill out the form online and print it, if that is more convenient.

6. Scholarship recipients will be notified in early April 2010; recipients are ex­pected to attend the Beta Alpha Psi/Accounting Association Spring Awards Banquet which will be held in late April (details to be posted at a later date).  More details will be pro­vided in the award letters.
